Cancellation Policy

Appointment Cancellation and Rescheduling Policy 1. Cancellation/No Show Policy for Appointments or Classes We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you maybe preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment book. If an appointment is not cancelled or rescheduled at least 24 hours in advance, you will be charged a forty-dollar ($40) fee, this will NOT be covered by your insurance company. 2. Scheduled Appointments We understand that delays can happen however we must try to keep the other patients and doctors on time. If a patient is 15 minutes past their scheduled time, we will have to reschedule the appointment. 3. ***WE REQUIRE THAT ALL PATIENTS PROVIDE A CREDIT/DEBIT CARD TO BE FILED in order to pay cancellation or no-show fees. Patients who prefer to not provide this information are given the option of pre-paying for their co-pay or other fees prior to booking their appointment. This may be done in person or by calling the office at (813) 530-9440. Options to cancel or reschedule appointments: 1. Call the office at (813) 530-9440 and choose option #1. If you are directed to voicemail, leave a message with your name and date and time of the appointment you wish to cancel or reschedule. 2. Text the office at (813) 530-9440 your name and date and time of the appointment you wish to cancel or reschedule. 3. Email the office at info@balancespinerehab.com your name and date and time of the appointment you wish to cancel or reschedule.
